Key Responsibilities

• Leading & supervising clinical nursing team and supports delivering advanced nursing care.
• Assess, plan, and evaluate comprehensive care cycles for patients transitioning from traditional ICUs.
• Collaborate closely with clinical staff, physicians, respiratory therapists, and leadership to drive positive patient outcomes.
• Works collaboratively to keep CCO & CEO apprised of situations of that may place hospital at risk.

Job Requirements

• Education: Graduate of an accredited school of nursing.
• Licensure: Active, unencumbered Indiana Registered Nurse (RN) license.
• Certifications: Current BLS and ACLS certifications.
• Experience: Prior nursing experience in a high-acuity setting (e.g., ICU, Step-down, CCU, or high-acuity LTAC) is highly preferred. Willing to train  and precept self-motivated RNs passionate about critical care.
• Skills: Strong clinical assessment capabilities and prior leadership experience; engaged, self-motivated clinical leader; professional fluency in verbal and written English.

About the Opportunity

We are seeking a motivated and relationship-focused full-time Clinical Liaison who is a Registered Nurse (RN) , Respiratory Therapist (RT) , Licensed Practical Nurse (LPN) or Physical Therapist (PT).

This role combines clinical expertise, relationship development, and patient advocacy. The Clinical Liaison helps appropriate patients transition to AMG for continued specialized hospital-level care.

The position primarily covers Chicago area hospitals and requires daily travel to hospitals, referral sources, meetings, and prospective referral partners.

What You’ll Do

• Evaluate referred patients for clinical appropriateness
• Educate physicians, case managers, discharge planners, and other healthcare professionals about AMG’s services
• Develop and maintain strong relationships with referral partners
• Identify patients who may benefit from long-term acute care hospital services
• Support timely communication and a smooth referral and admission process
• Help increase appropriate referrals, admissions, and hospital census
• Manage and grow an assigned territory

• Contributions to the Clinical Liaison team’s overall census goals

The Telemetry Monitor Technician - Unit Secretary accurately interprets cardiac rhythm strips every 4 hours including rate, rhythm, PR interval, QRS, QT, signature and title.  Immediately reports changes in patient’s rhythm to Primary Care Nurse.  Posts copy of rhythm change in patients record.  Documents arrhythmias and/or change in cardiac rhythm on report sheet and gives complete/accurate report to oncoming technician.  Properly operates monitor system.  Properly operates speaker system and phone system.  Continuously runs a strip during a code blue.  Properly completes emergency calls for staff.  Tracks monitored patients at all times.  Keeps charts prepped with labeled rhythm strips.  Maintains nutritional supplies.  Counts prior to ordering and maintains pre-set par limits.  Keep patient label book up to date.  Places completed rhythm sheets in patient charts. Provides indirect patient care in the medical surgical setting.  Meets the communication needs of the patient/family, departmental staff and medical staff.  Prepares and compiles records in the Medical Surgical Unit.  Initiates directions from physician and nursing staff.  Participates in performance improvement activities.  Maintains regulatory agency requirements, nursing and hospital policies and procedures and standards.  Performs other duties as needed or directed by supervisor.
